I couldn't find documentation of this.

How do I upgrade my bootstrap?  Running 9.2.1 and I seem to have Bootstrap 3 components.

Does a skin have to install this?

      Yes.

If you upgrade your skin to Bootstrap 4, you should be good to go. The only exception should be having Bootstrap3 stuff in any modules.

As Joe kind of mentioned, Bootstrap and any other similar presentation layer framework isn't really dependent on DNN. Rather, this is something that is or is not included in the theme (a.k.a., skin) that's installed into the DNN website. The modules you use either need to be compatible with the presentation framework you use, or you'll want to choose/use modules that include a template feature so that you can do that yourself. This is also a good reason to use a structured content module for most of your "static" content (e.g., OpenContent). :)
