That dialog is accessible when duplicating a page on the latest version of DNN. You need to go to the Modules tab under Advanced to find it. It's possible it isn't in 9.1.1, I'm not certain, but it is in current versions.

For clarity, the Duplicate option is available in the menu in the page listing
