Format HTML Visualizer for Reports Module

Congrats on posting!

We are running the latest releae of the Reports Module on DNN 09.13.9 and are trying to make sense of the use of the HTML Visualiser.  I cannot locate a sample template that will run correctly.  I have a SQL query that suplys correct information using the Grid Visualizer;l but would like rto use the HTML Visualiser.

What is the complete format or can you provide a sample template.  The snippet in the current documentation leaves a lot to be desired.
