Azure - Multiple Instances

We have a site that is running 2 instances (scale out) within Azure web app, that when updating content isn't flowing through on the other instance. This occurs with new content/module, editing an existing module and adding a new pages.

If you restart the application or change any of the cache settings it appears to flow through the new content, but then any updates after this are not.

Has anyone come across this and have a solution, or are there any recommendations for running a site with multiple instances.

Would you be able to better describe how you normally ensure the content is published/copied on both sites?  It sounds like you may have a custom feature built-in to do this for you, or it's a web farm, or it's a data transfer routine, or something else.  That information could help us direct a more specific answer for you.
