DNN version is v.09.01.01.Error as below. Anyone can advise. Thanks.

This is just a random error message and is unrelated to uploading files, and anything to do with files. What are you trying to do, exactly? A step-by-step description would be helpful for any of us to help you. :)

1) After login, change to edit mode by clicking at the pencil.

2) Add module, choose HTML to create new file or image.

3) Click at pencil to select edit content to create/ upload new file

4) Upon clicking the browser server, can see all saved image/ files. Upload icon has no response even though it was clicked.

5) This is where the issue persists. Add file is not able to click as there’s no ‘valid’ button to press before uploading. Previously, there will be a prominent button + Add File to add new file or image.

I'm sorry, but I think you're missing one or more steps.  Are you trying to upload something in the HTML editor of the HTML module?  What kind of file is it?

Also, using the HTML module is overkill if all you want to do is display a bit of media.  You may want to consider using the Media module instead.  This is all it does, which is especially great if you have others editing the site that you may be concerned with.
